GLP-1 Medication Cost Without Insurance: 2026 Guide

You searched the price. You saw the number. Somewhere between $1,000 and $1,500 per month, depending on which pharmacy and which brand-name product. Then you probably closed the tab.

That sticker shock is real, and it stops a lot of people from pursuing a treatment that their provider might otherwise recommend. This page explains why brand-name GLP-1 medications cost that much, what the compounded alternative costs and what that price actually includes, and how payment options like FSA and HSA can reduce your out-of-pocket expense.

Why brand-name GLP-1 medications cost so much

The short answer is patent protection and manufacturer pricing strategy. Longer answer below.

Brand-name GLP-1 medications are protected by multiple overlapping patents, which give manufacturers exclusive rights to produce and sell them for years after approval. During that exclusivity window, the manufacturer sets the price without competition from lower-cost alternatives. This is standard in pharmaceutical markets, not unique to GLP-1 medications.

In the US specifically, Medicare was prohibited from negotiating drug prices directly until the Inflation Reduction Act of 2022, which allowed limited negotiation starting in 2026. Private insurance plans have more flexibility, but negotiated rates depend heavily on how much volume a plan covers, and coverage for weight management medications remains inconsistent.

The result: without insurance or a manufacturer savings program, retail cash prices for brand-name GLP-1 medications are in the $1,000-$1,500/mo range, as confirmed by GoodRx and publicly available pharmacy pricing data.

The compounded medication alternative

When brand-name medications are on the FDA’s drug shortage list, federal law permits licensed compounding pharmacies to prepare compounded versions of those medications. This provision exists to ensure patients who need a medication can continue accessing it during shortage periods.

Both tirzepatide and semaglutide were added to the FDA drug shortage list, which created the legal basis for widespread compounding of these medications. As of early 2025, the FDA removed semaglutide from the shortage list, and tirzepatide followed. The post-shortage regulatory picture is still developing, and availability of compounded GLP-1 medications is subject to change as FDA policy evolves.

Important: Compounded medications are not FDA-approved products. They are prepared by licensed US-based compounding pharmacies and have not been independently evaluated by the FDA for safety, efficacy, or quality. They are not the same as, and should not be considered equivalent to, branded versions of these medications. All prescriptions require evaluation by an independent, licensed healthcare provider. Not all patients will qualify.

Transformation Health pricing: what you actually pay

Transformation Health offers four program tiers. Every plan is all-inclusive.

ProgramMonthly PriceFormulation
Semaglutide$249/moInjectable
Semaglutide$279/moOral
Tirzepatide$339/moInjectable
Microdose GLP-1/GIP$199/moInjectable or oral

These prices cover everything in the section below. There are no separate bills.

What “all-inclusive” means

A lot of telehealth providers advertise a low monthly number, then charge separately for the things that make that number possible. Provider consultations, required lab panels, ongoing monitoring check-ins, and shipping often get added on.

When we say “all-inclusive,” we mean the following items are covered in one flat monthly price:

Compounded medication. Your medication is prepared by a licensed US-based compounding pharmacy and shipped directly to your door. The cost of the medication itself is included in your monthly price.

Provider care. An independent, licensed healthcare provider reviews your intake, evaluates your health history, determines whether a prescription is clinically appropriate, and monitors your progress through regular check-ins. There is no separate consultation fee.

Lab work. Required blood panels are included at no additional cost. Labs are drawn at Quest Diagnostics or Labcorp locations near you. No separate lab bills, no insurance claims to file.

Medical weight loss coaching. Nutrition guidance, exercise recommendations, and lifestyle strategies are part of your plan from day one.

24/7 message support. You can contact your clinical care team at any time with questions about your medication, side effects, or treatment progress.

Free shipping. Discreet, temperature-controlled delivery to your door. No delivery charges.

Price lock. Your monthly rate is locked from the day you enroll. It does not increase as long as you remain a member.

Nothing about the list above changes based on which plan you choose. The Microdose plan at $199/mo includes the same components as the Tirzepatide plan at $339/mo.

FSA and HSA eligibility

GLP-1 medications prescribed by a licensed provider for weight management are generally eligible for FSA (Flexible Spending Account) and HSA (Health Savings Account) reimbursement. The IRS has not issued explicit guidance specific to GLP-1 weight management prescriptions, but these medications are prescribed as medical treatment by a licensed healthcare provider, which generally meets the definition of a qualifying medical expense under IRS Publication 502.

We accept FSA and HSA debit cards directly at checkout. No reimbursement forms, no claims to submit. The full all-inclusive monthly cost can be paid with your FSA or HSA card.

We do not currently accept American Express.

Check with your plan administrator for your specific plan’s coverage rules. FSA accounts are use-it-or-lose-it annually; HSA funds roll over indefinitely, which makes HSAs particularly useful for ongoing prescription costs.

How the price compares

Brand-name GLP-1 medications retail for $1,000-$1,500+/mo without insurance at most pharmacies. Provider visits, labs, and monitoring are typically billed separately.

Traditional in-person weight loss clinics often charge $500-$800+/mo for medication alone, with additional charges for provider consultations ($100-$300 per visit) and lab panels ($200-$400 each).

Our compounded programs run $199-$339/mo with medication, provider care, labs, coaching, and shipping included. Compounded medications are different products from branded medications. They are not FDA-approved. The lower price reflects the compounding model, not clinical equivalence.

Brand-name semaglutide and tirzepatide products are registered trademarks of their respective manufacturers. Transformation Health is not affiliated with or endorsed by any manufacturer of branded GLP-1 medications.

How much does semaglutide cost without insurance?
Brand-name semaglutide medications retail for $1,000-$1,500/mo without insurance at most pharmacies. Compounded semaglutide from a licensed US compounding pharmacy, as offered through Transformation Health, starts at $249/mo (injectable) or $279/mo (oral). Compounded medications are not FDA-approved and are different products from brand-name medications.
What does the monthly price at Transformation Health include?
Everything. Your monthly cost covers your compounded medication, provider care, required lab work (drawn at Quest or Labcorp), medical weight loss coaching, 24/7 message support, and free shipping. There are no separate consultation fees, no lab bills, and no hidden charges.
Can I use my FSA or HSA for GLP-1 medication?
Yes. GLP-1 medications prescribed by a licensed provider for weight management are generally FSA and HSA eligible. We accept FSA/HSA debit cards directly at checkout. Check with your plan administrator for your specific plan's coverage rules.
Why is compounded semaglutide less expensive than branded versions?
Brand-name GLP-1 medications are protected by patents and priced by the manufacturer. Compounding pharmacies prepare medications from active pharmaceutical ingredients, which removes the brand premium. It also means the compounded version has not been reviewed by the FDA for safety, efficacy, or quality, which is an important distinction. All prescriptions require evaluation by an independent, licensed provider.
Which plan is right for me?
The right plan depends on your health history, current BMI, weight management goals, and whether you prefer an injectable or oral medication. An independent, licensed provider will review your intake and recommend the appropriate formulation, if a prescription is clinically appropriate.

Our Platform & Your Independent Medical Team

Transformation Health is a modern technology platform designed to connect you with high-quality, convenient telehealth services. We facilitate your access to medical care; we do not provide the medical care ourselves.

All medical services are provided by independent, U.S.-licensed healthcare providers. These dedicated professionals are responsible for all clinical decisions, including diagnosis, treatment, and prescribing. Your confidential doctor-patient relationship is established directly with your independent provider to ensure your care is compliant, personalized, and focused on your unique health goals.

Understanding Your Medication: Compounded Formulations

The medications available through this platform are prepared by U.S.-based, state-licensed compounding pharmacies. These facilities are highly regulated and must adhere to standards set by their respective State Boards of Pharmacy.

Compounding allows pharmacists to create personalized medication formulations to meet specific patient needs, such as providing an alternative for a medication that is in shortage or creating a formulation without an ingredient a patient is allergic to.

It is important to understand that, as is the case with all compounded medications, these specific formulations are not FDA-approved. The FDA-approval process is designed for mass-produced, branded drugs. Compounded medications (which may utilize salt forms like semaglutide sodium/acetate) are prepared for individual patients and do not undergo the same large-scale FDA review for safety and efficacy. Your licensed provider will determine if this type of medication is the appropriate treatment for you. Transformation Health is not affiliated with, nor endorsed by, the manufacturers of any brand-name medications mentioned (e.g., Ozempic®, Wegovy®, Mounjaro®).
