Why brand-name GLP-1 medications cost so much

Brand-name GLP-1 medications are protected by multiple overlapping patents, which give manufacturers exclusive rights to produce and sell them for years after approval. During that exclusivity window, the manufacturer sets the price without competition from lower-cost alternatives. This is standard in pharmaceutical markets, not unique to GLP-1 medications.
In the US specifically, Medicare was prohibited from negotiating drug prices directly until the Inflation Reduction Act of 2022, which allowed limited negotiation starting in 2026. Private insurance plans have more flexibility, but negotiated rates depend heavily on how much volume a plan covers, and coverage for weight management medications remains inconsistent.
The result: without insurance or a manufacturer savings program, retail cash prices for brand-name GLP-1 medications are in the $1,000-$1,500/mo range, as confirmed by GoodRx and publicly available pharmacy pricing data.
The compounded medication alternative
When brand-name medications are on the FDA’s drug shortage list, federal law permits licensed compounding pharmacies to prepare compounded versions of those medications. This provision exists to ensure patients who need a medication can continue accessing it during shortage periods.
Both tirzepatide and semaglutide were added to the FDA drug shortage list, which created the legal basis for widespread compounding of these medications. As of early 2025, the FDA removed semaglutide from the shortage list, and tirzepatide followed. The post-shortage regulatory picture is still developing, and availability of compounded GLP-1 medications is subject to change as FDA policy evolves.
Important: Compounded medications are not FDA-approved products. They are prepared by licensed US-based compounding pharmacies and have not been independently evaluated by the FDA for safety, efficacy, or quality. Nothing about the list above changes based on which plan you choose. The Microdose plan at $199/mo includes the same components as the Tirzepatide plan at $339/mo.
FSA and HSA eligibility
GLP-1 medications prescribed by a licensed provider for weight management are generally eligible for FSA (Flexible Spending Account) and HSA (Health Savings Account) reimbursement. The IRS has not issued explicit guidance specific to GLP-1 weight management prescriptions, but these medications are prescribed as medical treatment by a licensed healthcare provider, which generally meets the definition of a qualifying medical expense under IRS Publication 502.
