After your provider reviews your intake and approves the prescription, a licensed compounding pharmacy prepares your medication and ships it to your address. Here is how the timeline works for a first shipment:
Provider approval (0-1 days)
Your intake form is reviewed by an independent, licensed provider. If the prescription is medically appropriate for your health history, the approval is sent to the licensed compounding pharmacy.
Pharmacy preparation (3-7 business days)
The compounding pharmacy receives your prescription and prepares your medication vial(s) with your specific dose. They package it with temperature-controlled insulation and prepare the shipment.
Shipment in transit (2-3 business days)
Your package ships via insulated, temperature-controlled carrier. You'll receive a tracking number so you can monitor delivery in real time.
Delivery to your door (7-14 days total)
Most patients receive their first shipment within 7-14 days of starting the intake process. Residents of AR, DC, DE, MS, NM, RI, and WV require a live video consultation before the prescription can be written, which may add 1-2 business days to this timeline.
What is included in the shipment
Your medication arrives as a complete kit. You do not need to source syringes, swabs, or instructions elsewhere. Here is what you will find in your package:
- The medication vial(s) in insulated, temperature-controlled packaging
- Insulin syringes pre-sized for your prescribed dose volume
- Alcohol swabs for skin prep before injection
- Administration instructions with step-by-step photos and written guidance
- The prescription label with your name, specific dose, and injection frequency
- Sharps disposal information and a sharps container if applicable

Temperature-controlled packaging explained
Compounded GLP-1 medications are sensitive to temperature. Here is how the shipping process protects your medication from heat damage:
During transit: Medications ship in insulated boxes with gel packs or cold packs. These maintain the temperature between 2-8 degrees Celsius[1], the refrigeration range required for stability. The insulation is designed to protect the medication even if the package sits in a warm delivery truck for several hours.
What you should know: Medications cannot be frozen. Freezing damages the medication and makes it unsafe to use. If you notice your medication has ice crystals or appears frozen when it arrives, do not use it.
Signs of temperature damage: If a medication has been exposed to excessive heat during delivery, it may show:
- Discoloration (the solution may appear darker or have a color tint)
- Cloudiness or visible particles in a normally clear solution
- Separation or layering (normally clear solutions appearing cloudy)
If you notice any of these signs, do not use the medication. Contact support before injecting.
What if you are concerned: If your package arrived and sat in direct sun or extreme heat for an extended period, you can inspect the medication before using it. Look at the vial against a white background. A normally clear solution should be completely clear. If you see cloudiness, discoloration, or any particles, contact support.
What to do on delivery day
Your package is now on your doorstep. Here is the step-by-step process to get it safely stored and ready to use:
Inspect the package for damage - Look at the box exterior for tears, dents, or wet spots. If the box is visibly damaged, open it carefully and assess whether the contents appear intact.
Check the temperature - Feel the gel packs or insulation. They should feel cool but not frozen. If everything feels properly chilled, your medication has maintained temperature during transit.
Transfer medication to the refrigerator immediately - Remove the medication vial from the insulated package and place it directly in your refrigerator (2-8 degrees Celsius)[1]. Do not leave it on the counter.
Do not freeze - Make sure your refrigerator is not set so cold that it freezes items. GLP-1 medications cannot tolerate freezing temperatures. Store your vial on a refrigerator shelf, not in the freezer compartment.

Refills and monthly shipments
After your first shipment, subsequent doses follow an automatic monthly schedule based on your provider’s prescription.
How it works: Your medication ships automatically each month on the date your prescription specifies. Most patients are on a monthly cycle, meaning a new vial arrives every 30 days.
You will be notified: Before each refill ships, you will receive an email confirming the shipment date and providing a tracking number so you know when to expect the package.

Your provider can adjust: If your provider recommends a dose increase, decrease, or change in formulation (for example, from injectable to oral), the next shipment will reflect that adjustment.
If something goes wrong
Shipping is reliable, but problems can occasionally occur. Here is what to do for the most common scenarios:
Package not received:
- Check the tracking number provided in your shipment confirmation email. If the package shows as delivered but you have not received it, check with neighbors or household members.
- If the package is genuinely missing, contact Transformation Health support immediately with your order number and tracking information. We will file a claim with the carrier and either reship your medication or issue a refund.
Medication appears damaged or compromised:
- Do not use the medication.
- Contact support immediately. Describe what you see, such as cloudiness, discoloration, or visible particles.
- We will either reship a replacement or issue a refund.
Temperature concern during shipping:
- If your package was left in extreme heat (for example, in a hot car or direct sun), inspect the medication before using it.
- If the medication appears clear and normal, it is safe to use. If you see any signs of heat damage, contact support before injecting.
Wrong medication or incorrect dose:
- Contact support immediately. Do not use medication you are unsure about.
- We will reship the correct medication.
Missing supplies:
- If your package is missing syringes, swabs, or instructions, contact support. We can either reship a supply kit or direct you to where you can obtain syringes locally if you need them urgently.
Shipping to all US states
Transformation Health ships to all 50 states. Residents of some states are required by law[2] to complete a live video consultation with a provider before a prescription can be written. These states are:
