Here is what typically happens:
Most formularies categorize GLP-1 by indication. Your insurer may cover semaglutide as a diabetes medication but specifically exclude it as a weight management treatment. These are treated as separate coverage categories, even when the active ingredient is identical.
Prior authorization requirements are strict. Plans that do technically cover obesity medications often require documented prior treatment failures, specific BMI thresholds, co-morbidity documentation, and multi-step appeals. Many patients qualify clinically but do not get past the administrative process.
Some plans exclude obesity medications entirely. Employer-sponsored plans have wide latitude in their benefit design. Many smaller plans and self-insured employer plans simply exclude anti-obesity medications from their formularies altogether.
Medicare does not cover GLP-1 for weight management. Medicare Part D covers GLP-1 medications for type 2 diabetes. As of early 2026, Medicare does not cover them for weight management alone. The TREAT Act has been proposed in Congress to expand Medicare coverage for obesity treatment, but it had not passed as of 2026.

A note on the Microdose Maintenance program
